How to register
If you wish to register as a patient at our practice, you need to complete our registrations forms. These are available By:
- calling into either of our sites
- By downloading and returning the following forms – Children Under 15 Welcome Pack | Adults 15 Plus Welcome Pack
- Download form GMS1 ensuring you sign the form and return this to us. Click Here
Whichever method you choose you will need to call into one of our sites to sign your forms etc.
If you need assistance one of our Patient Services Assistants Team will be happy to talk you through the process.
The medical questionnaire is required because it can take a considerable time for us to receive your medical records.
You are required to provide us with proof of your address.
You are also required to provide us with proof of identity but this does not have to be photo ID. Please note that is fraudulent to falsify your identity. The Practice has links with the NHS Anti-Fraud Team and is required to report any suspicions relating to identity misuse.
Allocation to a named doctor
If your registration is successful you will be notified in writing of your allocation to a named GP.
Patients have the right to express a preference to receive services from a particular doctor or professional group and the practice will endeavour to comply with any reasonable preference expressed. Nevertheless, the practice has the right to refuse such a request where it has reasonable grounds to do so. This includes equity of distribution of patients across GPs.
Overseas visitors
If you are visiting from overseas you may not be entitled to free treatment. It partly depends on whether your home country has reciprocal arrangements with the UK.
Keeping the Practice updated
Please let us know of any changes to your personal circumstances including name and address.
