- RECORD – Record those needs communication needs in a standardised way
- ALERT– Highlight a patient’s record, so it is clear that they have information or communication needs, and clearly explain how those needs should be met.
- SHARE – Include information about a patient’s needs with other NHS and adult social care providers when they have consent or permission to do so
- ACT – Make sure that people get information in an accessible way and communication support if they need it.
Patient Transport
If you’re going to hospital for an appointment, test or treatment, you’ll normally be expected to make your own way there.
You could ask a family member or friend to take you.
However, you may be eligible for non-emergency patient transport under certain circumstances.
Find out if you are entitled to NHS free hospital transport by phoning the NHS Patient Transport Advice Centre on 01278 727444 between 8.30am – 6.30pm Monday to Friday.
